Health Care Plan
A Health Care Plan is required for the conditions below. Plans must be returned with the child's photo attached but need not be signed by the Doctor.
- Severe asthma, Type 1 diabetes, epilepsy and anaphylaxis
- Any student who is diagnosed as being at risk of an emergency
- Any student who required the administration of specific health care procedures
Action Plans
In addition to a HealthCare Plan, Action Plans need to be provided for all Children with asthma, allergies or anaphylaxis. It is a requirement to use ASCIA Action Plans and Asthma Australia Action Plans. These must be signed and dated by a Doctor with a photo attached. All plans must be originals.
